Donation Request Process

  1. Go to Request Forms and choose the appropriate form for your project.
  1. Complete and submit the form online:
    • Enter your email address and password in the spaces provided. Note: If this is your first time submitting a request via Medtronic’s new online request process, choose the “I’m a new online applicant” button. The password you enter will become the password for your account.
    • Complete the eligibility section
    • Complete the request section
    • Acknowledge the certifications at the end of the request
    • Attach supporting documentation
    • Submit
  1. An automatic acknowledgement will be sent to the email address listed on the form you provided when you logged into your account.
  1. Your request will be automatically logged into the Medtronic donations system and will then be available to our grant administrators for processing. If you want to review your submission at any time, go to “My Account,” enter your email address and password, then select the desired request.
  1. Medtronic will review your request and make a decision to approve or deny the request to ensure compliance with Medtronic’s U.S. Business Conduct Standards and the AdvaMed Code of Ethics. The review process may take up to 60 days. If your request is approved, Medtronic will provide notification to you and will send a check to the address you provided on your form.
