Empowering Patient Communities

Overview

Patients with chronic health conditions often seek information and a means to act. Through PatientLink grants, we partner with national and international patient organizations that build awareness, educate and advocate on behalf of patients and their families to improve the lives of people with chronic diseases. PatientLink draws on the knowledge of Medtronic and its commitment to improving the lives of patients. We offer PatientLink grants in Canada, Europe, Japan and the United States.

Funding Priorities

In partnership with our PatientLink organizations, we aim to impact:

  • Awareness of chronic disease, prevalence, risks and resources in the community regardless of gender, race, ethnicity or disability
  • Education of patients and families to be informed and active in managing their condition
  • Advocacy ranging from personal health advocacy and patient representation on the boards of patient organizations to shaping government health policy

PatientLink grants are awarded for specific projects, programs and capacity-building activities.

GUIDELINES-Canada/Europe/Japan

Eligibility

Applications to the PatientLink program are by invitation and letter of inquiry. To be eligible for participation, a patient advocacy group should have a:

  1. Focus on chronic diseases or issues where Medtronic has expertise and commitment (e.g., Cardiac/Vascular, Diabetes, Gastroenterological/Urological, Movement Disorders, Pain, Spasticity, Spinal, Health Disparities)
  2. National scope with proven programs and/or chapters operating in a majority of regions or across countries
  3. Governance structure that includes patients and a board that reflects a diversity of professionals
  4. Proven growth and a strong strategic plan
  5. Non-profit association or charity status

Being preselected as a potential PatientLink grantee does not guarantee that a grant will be awarded, as projects are awarded on their individual merit.

Size and Terms of Grants;

  • PatientLink grants typically range from $10,000 to $40,000 based on their scope and impact. Grant awards at the higher end of the range are limited to organizations with proven results from past PatientLink funding and with the capacity to implement larger projects.
  • Grants are for 12 months. Grantees should describe how a project will be supported after the term of the grant.
  • A PatientLink organization is eligible to apply for a new grant in future years, assuming that the previous grant was completed as planned and a final report is submitted. Previous grantees may request multi-year funding. Funds based on multi-year commitments are released annually and are dependent on a yearly evaluation.

Eligibility

  1. Focus on chronic diseases or issues where Medtronic has expertise and commitment (e.g., Cardiac/Vascular, Diabetes, Gastroenterological/Urological, Movement Disorders, Pain, Spasticity, Spinal, Health Disparities)
  2. National scope with proven programs and/or chapters operating in a majority of states, regions or across countries
  3. Governance structure that includes patients and a board that reflects a diversity of professionals
  4. Proven growth, a strong strategic plan and an organization that is in the top 10% of peer group (i.e., organizations with staff and programs that have grown beyond $500,000 in annual budget)
  5. 501(c)(3) IRS determination letter verifying status

Being preselected as a potential PatientLink grantee does not guarantee that a grant will be awarded, as projects are evaluated on their individual merits.

  • PatientLink grants typically range from $25,000 to $100,000 based on their scope and impact. Grant awards at the higher end of the range are limited to organizations with proven results from past PatientLink funding and with the capacity to implement larger projects.
  • Grants are for 12 months. Grantees should describe how a project will be supported after the term of the grant.
  • A PatientLink organization is eligible to apply for a new grant in future years, assuming that the previous grant was completed as planned and a final report is submitted. Previous grantees may request multi-year funding. Funds based on multi-year commitments are released annually and are dependent on a yearly evaluation.

GUIDELINES-US Leadership Development Award »

GUIDELINES-US Leadership Development Award

The goal is to help high-potential PatientLink organizations increase their capacity through effective leadership, thereby ensuring the availability of quality resources that achieve the overall PatientLink goal of empowering patient communities. Specific interest centers around supporting strategy development including collaborations and mergers, and training opportunities for executive management and board members of PatientLink organizations.

Areas of Interest

The Medtronic Foundation gives priority to leadership development programs that include board members and address key issues such as:

  • Collaboration leadership training and development of core team members across the collaborating groups.
  • Merger strategic planning and development of implementation plans.
  • Succession planning for future leader transitions and management realignment.

Eligibility

Application to the PatientLink Leadership Development Award program should first be discussed with the Medtronic Foundation staff. Eligible programs:

  1. Current PatientLink grantees.
  2. Programs should focus on developing Executive Team, Board Members and Senior Leadership Team of the PatientLink organization or collaborating organizations.
  3. Organizations receiving awards are required to contribute at least 25% of the cost of the proposed program.

Being a PatientLink grantee does not guarantee that a grant will be awarded, as projects are evaluated on their individual merits.

Size and Terms of Grants

  • Awards range from $1,000 to $20,000 depending upon the proposed plan.
  • Awards are for 12 months.
  • Only two Leadership Development Awards will be given to a PatientLink organization in a 5 year period.
  • Awardees will be expected to provide an evaluation or final report of their program.

Grant Deadlines

  • Grant applications are accepted between May 1 – January 15.
  • Review of a grant application can take up to three months.
