Prescription Form for the Meniett Device for Ménière’s Disease
A prescription is required to order the Meniett® device for Ménière's disease. All ENT (ear, nose, and throat) specialists and ear specialists (otologists and neurotologists) can prescribe the Meniett device. Download a prescription form for your physician.
Sample Letter of Medical Necessity
If your insurance carrier doesn’t automatically reimburse for the Meniett device, our Therapy Access Department may be able to help you get coverage. Please call us at (877) 966-3350 or e-mail us for more information. Also, consider asking your doctor for a letter of medical necessity to give your insurance provider— this may help you obtain reimbursement.
