Suspected Epilepsy and Asystole


BACKGROUND

For over six years, a 38-year-old women suffered multiple blackout episodes with seizures. She had no history of arrhythmia or cardiovascular disease. However, with blackouts her quality of life suffered and she:

  • Lived in fear of her next syncopal episode
  • Could not drive or exercise
  • Felt a negative impact with respect to her interpersonal relationships

WORKUP

The patient underwent numerous tests to diagnose her problem, including:

  • ECG, Holter monitoring, external loop recorder monitoring, tilt table testing, and EP studies – inconclusive
  • EEG after sleep deprivation and MRI – without diagnosis

The patient was placed on an anti-seizure medication for 12 months and experienced 6 additional episodes. At that point the patient was:

  • Referred back to a cardiologist
  • Implanted with a Reveal ILR

CONCLUSION

After implantation of the Reveal ILR, the patient experienced another episode:

  • The recording (below) showed normal sinus rhythm (NSR)/sinus bradycardia (SB) followed by 25 seconds of asystole, then NSR/SB again

  • The patient’s cardiologist made a diagnosis and implanted a dual chamber pacemaker
  • The patient still feels occasional dizziness, but she has not experienced another episode since diagnosis
