WHAT IS HEALTH MONITORING?

Health monitoring uses technology to monitor your health status at home and send information to your care team. Your care team can review this information and use it to help you manage your care at home.

The Medtronic Care Management Services health monitoring program may help you learn more about your health and communicate with your care team.

Daily health monitoring may limit unnecessary visits to the hospital. It’s our goal to give you peace of mind that your health is being monitored by your care team every day.

WHO CAN BENEFIT FROM HEALTH MONITORING?

If you have one or more chronic conditions, you’re not alone. More than half of all people living in the United States have at least one chronic condition, and 30% live with more than one chronic illness, such as heart failure, diabetes, or COPD.1

If you have one or more chronic conditions, it may be hard to understand and manage your health. A health monitoring program may help you achieve your health goals and find additional support after a hospital visit or in your day-to-day routine.

HOW DOES HEALTH MONITORING WORK?

Health monitoring works by collecting health information each day. This information is sent to your care team to review. Medtronic Care Management Services offers health monitoring programs which may be available through your healthcare provider or health plan.

Here's how it works:

YOU COMPLETE A DAILY HEALTH CHECK

You’re tasked with completing a health check using a tablet, tabletop device, or phone chosen by your healthcare provider or health plan. Each day:

  • You measure and collect vital signs such as weight or blood pressure.
  • You answer questions about your health status. The program shares this information with your care team. 
How health monitoring works for the patient

YOUR CARE TEAM REVIEWS

Your health check information is sent to Medtronic Care Management Services. Your care team reviews the information. If they see a notable change to your day-to-day health, the following may occur:

  • Your care team may contact you to learn more about your current health status.
  • Your care team may alert your doctor to help determine if your health requires attention.
  • Your doctor may reach out to you to make a change in your care plan.
