Insurance Coverage – Insulin Pumps
Medicare and most insurance plans provide coverage for insulin pump therapy and diabetes testing supplies. If you have questions about whether you may be covered for an insulin pump, ask your plan’s representative.
If you or a loved one is unable to work, take steps as soon as possible to find out what can be done to make up for any loss of family income. If you (or your loved one) have disability insurance, begin applying for benefits. Consider what other financial adjustments may also be needed.
Medtronic Diabetes has insurance specialists who will work with you, your doctor, and your insurance provider to check your coverage. They can negotiate with the insurance provider and will assist you each step of the way, from getting a prescription for a pump to getting reimbursed.
To get started with insulin pump therapy, you will need to fill out the Assignment of Benefits form and the Health Questionnaire at right.You will also need to ask your doctor for a prescription.
Prescriptions for ongoing insulin pump supplies need to be renewed every year and faxed to (866) 874-3150 to avoid unexpected supply delays.
