The COVID-19 Virtual Care Evaluation and Monitoring solution is no longer available. If you’re concerned about yourself or a dependent, and would like to evaluate potential COVID-19 symptoms, please use the self-assessment tool available through the Centers for Disease Control and Prevention (CDC). To access the CDC symptom checker:
 

• Go to cdc.gov/coronavirus
• Select “Self check symptoms” from the menu on the right
