If you are advocating to improve health outcomes for others, and / or contributing to innovation that improves the lives of people, and / or taking actions to make your dreams for a better world, a reality – we look forward to receiving your application for the Bakken Invitation. Applications close on July 18, 2018.

Please see below for more information on eligibility criteria for the Bakken Invitation. For the applicant to be eligible for consideration, the Application must be completed no later than 11:59:59 PM Central Time on July 18, 2018.

The application takes about 30 minutes to complete.


eligibility criteria

To be eligible for the Bakken Invitation, applicants must:

  • Be at least 14 years or older at the time of application with consent from a parent or legal guardian for applicants under the age of 18.
  • Have overcome a health challenge, earned “extra life”, with the help of medical technology and used that time to improve the life of others through advocacy for health, innovation for health or through selfless charity and service for their community.
  • Be, as the result of a qualifying medical technology, managing a qualifying chronic disease or have had their quality or quantity of life extended.
    • A qualifying chronic disease includes heart disease, stroke, diabetes, chronic pain, spinal or neurological disorders, gastrointestinal and urological disorders, obesity, or cancer.
    • A qualifying medical technology* includes medical device therapies and surgical procedures – including a pacemaker, implantable cardioverter defibrillator, spinal device, neurological device, insulin pump, heart valve, coronary, or thoracic stent, etc. If you have questions about whether or not your device, therapy, or procedure qualifies, please contact the Medtronic Foundation at rs.medtronicfoundation@medtronic.com
  • Be actively involved with a non-profit organization or a social enterprise for six months or more.

Former Bakken Invitation Honorees, Medtronic employees and direct family members are not eligible. Recipients will be chosen on a variety of factors, including geography, disease condition, past achievements and aspirations, and entry story.

Twelve Honorees will be chosen by a committee of patients, representatives of nonprofit organizations and Medtronic Foundation staff.


Notification

A committee of patients, representatives of nonprofit organizations and Medtronic Foundation staff will review candidates’ applications. Medtronic Foundation will notify the candidates selected to move on to the next stage of the selection process on or about August 15, 2018. Final application process consists of video interviews with Medtronic Foundation staff. 

The twelve selected Honorees will be notified by Medtronic Foundation on or around October 15, 2018.
