Program Guidelines

To be eligible for the Bakken Invitation Award, applicants must be:

  • At least 14 years old by December 31, 2017
  • Managing a qualifying chronic disease through a qualifying medical technology*
    • A qualifying chronic disease includes heart disease (including heart attack, cardiac arrest, coronary artery disease), stroke, diabetes, neurological, digestive and gastrointestinal, urological or spinal disorders, obesity, chronic pain, and cancer.
    • A qualifying medical technology includes medical device therapies and surgical procedures approved, cleared, or registered in accordance with the applicant’s country’s laws and regulations to manage or treat the chronic disease in accordance with the approved or cleared indications for use for the medical technology.
  • Giving their time and talent to improve the quality of life for others
  • Actively involved with a non-profit organization for 6 months or more

Applicants for the Bakken Award must complete and submit an application by July 24, 2017.

Medtronic employees and direct family members are not eligible.

*All applicants with eligible medical technology are welcome to apply, regardless of device manufacturer.

A selection committee comprised of nonprofit and Medtronic Foundation leaders will review applications and notify the 2017 Honorees in early October.

The Medtronic Foundation will host an Award recognition ceremony and workshop in Hawaii from January 10-14, 2018.
