Medtronic is committed to assisting hospitals with managing the complexities of product warranties. The warranty information on this page is specific to Canada’s Mechanical Circulatory Support (MCS) product line, which includes the HeartWare™ HVAD™ System.
Medtronic Canada has a defined standard warranty process for MCS products suspected to be functioning outside of normal tolerances. To initiate a warranty request, the following must occur within 30 days of the product malfunction date:
Medtronic has a dedicated Warranty Team to help answer warranty related questions/inquiries. The team can be reached via email at email@example.com or via phone at (888) 660-4616.
For timely processing, you will need to complete the PDF form electronically and provide an e-signature. Within 30 days of the product malfunction date, email the warranty claim form to the Medtronic Warranty Team at firstname.lastname@example.org. You may download and save the warranty form for future use and/or save the completed warranty form for your records.
