By placing an order for a new MiniMed 770G System upgrade as part of the Next Tech Pathway Program, I acknowledge and understand the following:

• There is no charge for the MiniMed 770G Insulin pump as part of this program. This offer is available until April 30, 2021.
• Continuous glucose monitoring (CGM) is sold separately. The MiniMed 770G system uses a different transmitter than the one you are using now. If you have an in-warranty transmitter at time of upgrading or you are currently part of a Medtronic CGM subscription program, you will be eligible to receive a transmitter at no charge with your MiniMed 770G.
• If you are new to CGM, we can help you get started and review options available. If requested above, a Medtronic representative will contact you over the next couple of weeks to discuss CGM Starter Kit options available or you can sign up for a 12 month CGM Subscription here.
• The remainder of the warranty on my current pump will transfer to my new MiniMed 770G insulin pump (i.e I will have the same warranty end date as my current pump).
• The remainder of the warranty on my current transmitter will transfer to my new transmitter
• The MiniMed 770G system cannot be returned once received.
• I acknowledge that I may keep my current insulin pump device but that it is unsafe to pass it on to someone else.
• I am aware that to use the apps with my MiniMed 770G system, I will need to have a compatible smartphone device and confirm I have reviewed compatibility, click here
• I acknowledge that I will follow the training instructions prior to starting on the MiniMed 770G system.
• I understand there is a training website that will be sent to me once my MiniMed 770G pump ships to help guide me through training options, including self-start, a virtual training class or a virtual 1:1 training with a certified trainer. I acknowledge that some self-directed pre-learning is required for the trainer-led options.

In order for us to be able to contact you, we need your consent to the following:

• Medtronic can communicate with you regarding our diabetes products by telephone, text, fax, mail and/or e-mail.
• That you have read and are aware of our privacy policy.
• That you grant Medtronic Canada ULC permission to send you updates on diabetes and diabetes technology..
• That Medtronic stores information, including personal health information, on systems located outside of Canada.
• You may revoke this permission at any time by emailing medtronicdiabetescc@medtronic.com
