By placing an order for a new MiniMed™ 780G System, I acknowledge the following: I understand the terms and conditions of the program, including:

The cost of the Pathway Program is $780
The actual length of time of the existing pump warranty will transfer and apply to the new MiniMed™ 780G system warranty at time of shipment (i.e. I will have the same warranty end date)
The MiniMed 780G system can’t be returned once received
I acknowledge that I may keep my current insulin pump device but that it is unsafe to pass it on to someone else.

This offer is available to customers who wish to replace their existing Medtronic or non-Medtronic pump with the new MiniMed™ 780G system provided there is a minimum of 6 months warranty remaining on it at time of shipment. The actual length of time of the existing pump warranty will transfer and apply to the new MiniMed™ 780G system warranty at time of shipment. One CGM Starter Kit is included with the new system (comprised of: Guardian Link (3) Transmitter and one box of 5 Guardian Sensor (3) glucose sensors). Limit to one transmitter per 12-month period while using CGM. This offer has no cash value. This Program is subject to change without notice. Program eligibility requirements will apply. The MiniMed™ 780G insulin pump system is indicated for use by patients age 7-80 years with Type 1 diabetes, whose totaldaily dose of insulin is 8 units per day or more. Some user interaction required. Individual results may vary.
