I would like to be connected to an Ambassador

I would like to be connected with a patient ambassador. I consent to Medtronic collecting, using, and disclosing my personal information, including health information that I provide during enrollment, participation in the Program, or communications relating to the Program, for the purpose of matching me with and connecting me to a patient ambassador, communicating with me regarding the Program, and evaluating and improving the Program through participant feedback and surveys. I understand that Medtronic may share my personal information with a patient ambassador to facilitate a connection. I understand that the information I provide to Medtronic will not be used by Medtronic for purposes outside of the Program without my prior written consent. I understand that patient ambassadors share personal experiences only and do not provide medical advice. I understand that patient ambassadors’ views are their own and do not necessarily represent Medtronic. I also understand that information I voluntarily share directly with a patient ambassador is outside Medtronic's control. I understand that I am responsible for adhering to appropriate personal information protection practices with any personal information voluntarily disclosed to me by the ambassador. I acknowledge that I have reviewed Medtronic's Privacy Notice.
