At Medtronic, we recognize we cannot fulfill our Mission alone. To “alleviate pain, restore health, and extend life,” we must support the many stakeholders in healthcare, including providers, medical societies, and patient groups. Part of our role is to provide healthcare related financial assistance for medical education and public education programs.

For healthcare-related requests, please complete the e-form linked below. Before submitting your request, please ensure that all required sections (six pages) are completed and all necessary documents are uploaded. You will also need your banking information to complete the Canada Electronic Funds Transfer form.

The information provided will be used to evaluate the scientific and educational merit of the fellowship program.
