Indications, Safety, and WarningsNASAL PACKINGIndications for useNasal Dressing, Epistaxis Packing, Sinus Packing, Ear Packing and Ear Wicks for epistaxis, post-operative surgery and trauma, which expands when moistened to assist in tamponade and hemostasis.ContraidicationsNone known.Adverse effectsIn rare instances the physiochemical conditions associated with nasal surgery, both with and without nasal packings, may present a risk of toxic shock syndrome (TSS). Warning signs of TSS include sudden fever (usually 102°F or more), vomiting, diarrhea, dizziness, fainting (or near fainting when standing up), and/or a rash that looks like a sunburn. Merocel nasal dressings exhibit no antimicrobial properties, they are not bacteriostatic towards pre-existing infections, nor do they prevent the occurrence of new infections. In the case of pre-existing infections it is recommended that appropriate treatment be instituted.Rarely, displacement and ingestion of a nasal dressing may occur. Merocel sponge is non-toxic and is considered safe to pass once it is completely in the alimentary tract. The patient should be monitored in the normal manner for the ingestion of any non-toxic material. Clinical confirmation that packing material has not been aspirated or is not in danger of being aspirated should be assessed. If aspiration should occur, prompt medical intervention is imperative with close patient monitoring and airway support pending the removal of the material.K.I.S.S (KENNEDY INTRANASAL SURGERY SPONGE)InsertionDecongest the nasal mucosa. Grasp the pack with bayonet forceps so the mark will lie anterior and superior. The pack is inserted (dry or wet/expanded) by passing between the inferior and middle turbinates so it lies within the posterior nose and nasopharynx (choana). If the position of the pack is in doubt, intraoral examination should be performed to ensure that the pack has not passed too far posteriorly.SutureSecure the string anteriorly by clamping it to the patient’s drape, by tying the strings from the pack in both nostrils over the columella or by attaching the button to the patient’s skin. Ensure that the pack is firmly secured in place before starting the operative procedure.RemovalSuction the nose free of clots and debris before removing sponge. Grasp with forceps and gently withdraw.
