Please complete this form to have the Medtronic Stay Connected service contact your patient to troubleshoot their monitor or explore their monitoring options.

Once you submit this form, Medtronic Stay Connected will attempt to call your patient within 1–2 business days. You will receive an email notification (at the email address below) when we have resolved the issue or after we have made three attempts to reach the patient and need more information or support from you to proceed.

If your patient needs immediate monitor assistance, please have them call Medtronic Stay Connected at 866-470-7709. Our staff is available Monday through Friday from 7:00 a.m. to 7:00 p.m. CT.

If this is a medical emergency, have them call 911 immediately.
