Selecting the right patients

Your patient may be indicated for the Symplicity™ procedure, which uses the Symplicity Spyral™ renal denervation (RDN) system, if they:


Have uncontrolled hypertension

Consider patients where lifestyle modifications and antihypertensive medications haven’t adequately controlled blood pressure. This may include patients who:

  • Still have high blood pressure despite taking medication
  • Are unable to adhere to or tolerate medication
  • Are on medication for comorbidities that may not be compatible with antihypertensive medication

Are willing to undergo an interventional procedure

Consider patients who opt for the Symplicity™ procedure following shared decision-making and an attempt at lifestyle modifications and medical therapy.

Examining society recommendations on patient selection for renal denervation

Society for Cardiovascular Angiography & Interventions (SCAI)3 recommends renal denervation for patients3:

  • With uncontrolled hypertension despite lifestyle changes and medication and are unable or unwilling to take additional medication
  • With resistant hypertension (> 130/80 mmHg) despite being on three medications
  • Who are willing to undergo renal denervation after shared decision-making
  • At higher cardiovascular risk

The 2025 hypertension guidelines published by the American College of Cardiology (ACC) and American Heart Association (AHA) recommend renal denervation4:

  • For patients with resistant or uncontrolled blood pressure†,‡ with stage 2 hypertension (office SBP ≥ 140 mmHg and DBP ≥ 90 mmHg)
    • Blood pressure not at goal despite taking ≥ 4 antihypertensive medications at optimal dosages
    • Unable to take antihypertensive medications at the optimal dosages or additional medications
  • Following shared decision-making, considering patient preference to support individualized care decisions

It is also recommended that institutions follow a team-based approach to care, which is important from patient selection through post-procedure follow-up.


Patient screening considerations

When screening patients for the Symplicity™ procedure, consider:

  • Assessing lifestyle modifications
  • Evaluating medications
  • Measuring out of office blood pressure (home or ambulatory)
  • Screening for secondary causes of hypertension (if indicated)

Once the procedure is done it keeps working 24 hours a day, like a pill you never have to remember to take.5,6
